These forms are available to print off from the links below. We ask that they are handed into one of the reception team to ensure that all the necessary information has been completed. You do not need photo identification, proof of address or proof of immigration status to register with a GP. Official guidance.
In summary, to register you should complete GMS1, new patient registration form, the data sharing consent forms. Photo ID is not required for GP registration.
